Rose Parade Medical Form – As part of the forms package students received, the Rose Parade Medical Form ask for the blood type of the student. If you do not know this information fill in “unknown” in the blank for blood type.
Legal Name Clarification – On the form that ask for your legal name please provide your name as follows: 1.) for all travelers 18 or older on December 27th, 2014 you must provide your name as it appears on a government issued identification card or driver’s license. You will be required to have this identification with you on travel. 2.) for all travelers under 18, provide your name as it appears on your Lakota West student identification card. You will need to bring your student identification card with you on the trip. If you do not have a student identification card, provide your name as it appears on your birth certificate and you will need to bring a copy of your birth certificate with you on travel.
